Why a backpack rather than a box
A traditional carrier is held in one hand at arm's length, and over any distance that becomes genuinely tiring while leaving you with one hand for doors, gates and a wallet. Distributing the weight across both shoulders removes all of that, and it keeps the cat closer to your body where the movement is steadier.
Motion is the underrated part. A carrier swinging from a hand rocks and jolts with every step, which is unsettling for an animal that has no idea what is happening. Worn on the back, the carrier moves with your torso and the ride is considerably smoother.
The visibility question
Cats divide fairly evenly on this. Some are reassured by seeing where they are going and will sit watching the world through the panel quite happily. Others find the flood of unfamiliar visual information overwhelming and do far better with the view covered.
You can test which yours is cheaply. Drape a light towel over the panel for the first outing and see whether the cat settles more or less than with it open. If covering helps, keep a cloth with the carrier permanently; it costs nothing and it makes the difference between a cat that travels calmly and one that does not.
Safety and ventilation
The internal clip is there to attach to a harness, not a collar, and it exists for the moment the carrier is opened at a destination. That is when cats bolt, and a cat loose in a vet's car park is the situation everything else is designed to avoid. Clip it before you unzip anything.
Watch temperature. A ventilated pod against your back is warmer than a carrier held at arm's length, because your body is a heat source and airflow across the panel is limited when you are walking. On a warm day keep journeys short, avoid direct sun on the pack, and never leave it closed in a car.

The cat must be able to stand, turn around and lie down. Measure your cat from nose to tail base and at the shoulder, and choose on those rather than on weight, since build varies far more than mass suggests.

Two indoor cats
My arm survived the vet trip
The old box carrier swung into my knee the whole walk down and by the time we arrived we were both fed up. On my back the weight sits still and he stayed quiet at the mesh panel watching the street go by. We took the large for a heavy boy of six kilos and there was room to spare.

Senior tabby
Zips need two hands
Both the top and front openings are useful and lowering a cat in through the top really is easier than posting one through an end hatch. The zip pulls are small though and stiff enough that you want a hand on either side, which is awkward when you are also holding a cat. Everything else has been right, particularly the structured base.

The internal clip caught a bolt
He made a run for the gap the moment the front unzipped at the surgery and the clip stopped him dead at about a foot. That alone has justified it in my head. The mesh on three sides means he can see what is happening, which seems to keep him calmer than the closed bag ever did.

Fussy rescue cat
Folds flat behind the wardrobe
We use it three or four times a year and the old one lived in the loft taking up a whole shelf. This collapses down to something I can slide behind the wardrobe door, and it pops back into shape without creasing. The base stays rigid when it is up, so it does not sag down onto her.

Fine carrier, warm against your back
Mesh on three panels does keep air moving and she was comfortable enough on the way there. The panel against my back is solid though, so on a warm day that side gets hot for both of us and I ended up stopping twice. It does the job and the straps are comfortable, I would just not want to carry it far in summer.

Straps take the weight properly
Five kilos is not much until it has been on one arm for twenty minutes. The padded shoulder straps spread it across both shoulders and I stopped noticing after the first street. Adjusting them is a single pull each side, which I could do with the bag already on my back.

Bengal cat
He settles once he can see out
In the old bag he howled from the front door to the car park without stopping for breath. With mesh on three sides he spends the journey watching, and the howling has dropped to a couple of complaints at the start. Same cat, same route, different bag.

Large is larger than I pictured
It is roomy inside, which is the point, but on a shorter person the large sits low and knocks the backs of my legs when I walk quickly. Loaded properly with the base flat it is fine. The top opening has made getting her in and out far less of a fight than the old front loader.

It suits cats and small dogs comfortably. Check they can stand and turn around inside; if they cannot, it is too small for them.
Airline rules vary by carrier and route, so check with your airline directly rather than assuming. Treat it as a car and vet carrier by default.
Sizes run from kittens to cats over 7 kg. The rule is that the cat must be able to stand, turn around and lie down comfortably, so measure rather than going by weight alone.
Check with your specific airline before booking. Cabin pet requirements vary considerably between carriers and change regularly, and no manufacturer can guarantee acceptance.
Go back to leaving it open at home with bedding and treats for a couple of weeks, then try short carries around the house. Covering the visibility panel helps a good number of cats.
